Having a foundation for effective case management is critical. This five-part series will cover solutions for many of the challenges faced by case managers today.
The series will first discuss key stakeholders and their integral role in the rounding process. Next, attendees will learn how to incorporate case management at all access points to their hospital or health system. This will be followed by the often challenging subject of how best to collaborate with physicians, especially high-volume ones such as hospitalists and ED physicians. The series then will review how to remain forward-looking in a professional practice. Finally, the series will discuss how to measure and how to report case management outcomes.

At the conclusion of these sessions, participants will be able to:
Part One
- Understand the differences between interdisciplinary rounds and other modalities such as huddles and teaching rounds
- Choose key members for the rounding team
- Identify the role of each team member through scripting and other tools
- Review a walking rounds checklist.
Part Two
- Discuss the Two Midnight Rule components and how they are positively impacted by access point case management
- Define best practices for case management in the ED, transfer center, and admitting office
- Identify ways to reduce readmissions and noncompliance at hospital access points.
Part Three
- Identify the key roles of the physician adviser, the hospitals,t and ED physicians
- Review case management specialty roles held by registered nurses and/or social workers
- Develop best practice strategies with key physician stakeholders to ensure optimal outcomes
- Identify opportunities for improvement at key locations throughout the hospital while collaborating with physicians and other case management team members.
Part Four
- Understand the continuum of care and why it is important to case management professionals
- Define transitions in care and discuss best practices
- Review how case management professionals can provide the integrating mechanisms for patients as they move across the continuum
- Apply strategies for engaging stakeholders across the continuum
- Ensure that patients and families are prepared for any transitions in care
- Describe the applications of case management in a variety of settings
- Define the three components of care transitions
- Discuss community case management and its importance to hospital case managers.

- Describe contemporary outcome measures for case management
- Develop the key performance indicators to measure case management outcomes
- Create a workable case management report card
- Understand how benchmarks can be applied to your case management report card.
